NEEDED FORMS FOR PRE-K3 & PRE K4 ADMISSIONS
Dear Parents,
Please complete the attached forms in full and return as soon as possible for Pre k3 and pre k4 admission. Once your child's file is complete, your child will be call for:
a pre k3 interview or pre k4 test
Please submit the following information with the completed applications:
1. $40.00 application fee (non refundable)
only cash, certified check or money order, payable to Our Lady of Grace School, will be accepted. (Cashier's Check or Money order Only!)
2. $40.00 Testing fee for Pre K 4 only (non refundable).

3. Birth Certificate
4. Complete health records as required by law for children entering school. Required Immunization, 4DPT, 33 OPV, 1 MMR, 1 HIB AND 3 HB
5. Original Baptismal Record, if applicable.
6. A wellness certificate completed and signed by your child's doctor.
7. Letter of Church affiliation for child and or parent.
8. Social Security Card
The Principal or teacher will interview each parent and child. All children entering Our Lady of Grace School for pre k4 will be tested A test date will be arranged upon submission of the completed application.
