Dear Parents,
Please complete the attached forms in full and return as soon as possible for admission.
Please submit the following information with the completed applications:
1. $40.00 application fee (non refundable)
only cash, certified check or money order, payable to Our Lady of Grace School, will be accepted. (Cashier's Check or Money order Only!)
2. $40.00 Testing fee for Pre K 4 only (non refundable).

3. Birth Certificate
4. Complete health records as required by law for children entering school. Required Immunization, 4DPT, 33 OPV, 1 MMR, 1 HIB AND 3 HB
5. Communion and Confirmation records, if applicable.
6. Most recent report card
7. Student information sheet in packet given at registration.
8. Original Baptismal records, if applicable.
9. Letter of Church affiliation for child and or parent.
10. Social Security Card.
Students applying for admission to Kindergarten thru grade 7 will be tested in Reading and Math. The school will inform you of a testing date once the principal review the child's file. If you have any questions, please do not hesitate to contact Ms. Reyes at 718 547-9918
